For patients using insurance, this membership helps ensure continuity of care between visits—making it easier to ask questions, coordinate care, and stay connected to your provider outside of scheduled appointments.

This out-of-pocket fee helps support the administrative, coordination, and communication services involved in managing insurance-based care, including care coordination and clinical support that occurs between visits.
