HOW SHOULD I PREPARE FOR MY INITIAL CONSULTATION? 

Register for a ChARM Portal, complete your New Patient Paperwork and complete Consent Forms. It is important to complete these forms as accurately and thoroughly as possible. Please include ALL details of any medications or supplements that you regularly take, including dosage. We require intake forms to be complete prior to scheduling your initial consult. Call the office to get started! 

 

If you plan to utilize health insurance, we highly recommend that you also complete our Verifying Insurance Benefits worksheet. We are a small practice and do not verify insurance benefits for potential patients. It is up to you to determine this information! 

DO YOU BILL HEALTH INSURANCE?

Yes. We are currently credentialed (in-network) with

  • Blue Cross Blue Shield

  • Aetna, Pacific Source

  • Allegiance

  • Cigna 

  • MT Health Co-Op

  • Missoula County.

Although we are in-network, this does not guarantee that the plan you have covers naturopathic physicians. If you plan to utilize health insurance, we highly recommend you call your insurance company to verify the details of your policy. Our front desk team as put together a tips and tricks handout to help with this process. Please give us call or email and can pass the handout along for your convenience. 

Please note, we do NOT bill insurance for any our IV Therapy and some other non covered services . If you have insurance and elect to receive one of these self-pay services, we will ask that you sign a Financial Agreement acknowledging that you will be paying out-of-pocket. In these instances, we will not provide a Superbill. We will give you a price quote upfront and make sure there are no hidden costs.

CAN A DOCTOR AT ARALIA HEALTH BE MY PRIMARY CARE PHYSICIAN?

Our doctor are specialized and do not serve as primary care providers. It is advisable to have a PCP external to Aralia Health. This arrangement allows our doctors to dedicate their office visits exclusively to Naturopathic Medicine specialties.​

Please note, we are unable to accommodate patients for acute office visits. We also do not offer after-hours consults. Therefore, we strongly encourage you to establish care with a PCP outside of Aralia Health.

WHAT IS YOUR CANCELLATION POLICY?

Aralia Health we strive to provide timely and effective care to all of our patients. When appointments are missed or cancelled without sufficient notice, it disrupts our ability to accommodate other patients in need of care. Therefore, we have implemented the following policy regarding missed appointments and short notice cancellations:

No Show Policy: A "no show" occurs when a patient fails to attend a scheduled appointment without prior notification. Patients who fail to show up for their scheduled appointment will be charged a fee equal to 50% of the cost of the scheduled appointment.

Short Notice Cancellation Policy: A short notice cancellation occurs when a patient cancels their appointment with less than 48 hours' notice. Patients who cancel their appointment with less than 48 hours' notice will be charged a fee equal to 50% of the cost of the scheduled appointment.

Payment of Fees: Fees for missed appointments and short notice cancellations will be billed to the patient and are due same day as the missed appointment or cancellation. Payment can be made by cash, check, or credit/debit card. If a card is stored on file under the patient’s account, it will be automatically changed.

Exceptions: We understand that unforeseen circumstances may arise that prevent patients from attending their scheduled appointments. In such cases, we encourage patients to contact us as soon as possible to reschedule their appointment. Exceptions to the no show and short notice cancellation policy may be made at the discretion of Aralia health. Examples are acute illness, emergencies and unsafe travel ways.

By scheduling an appointment with Aralia Health, patients acknowledge and agree to comply with the terms of our No Show and Short Notice Cancellation Policy. If you have any questions or concerns regarding this policy, please feel free to contact us.

HOW MUCH IS AN OFFICE VISIT IF IT IS NOT COVERED BY INSURANCE?

All fees are listed for payment at the time of service:

Naturopathic New Patient Appointment (99205+99417): $375 

Naturopathic Long Follow-up (99215+99417): $300 

Naturopathic Standard Follow-up (99215): $245

Naturopathic Focused Follow-up (99214): $175 ​

Orthopedic New Patient Appointment (99205): $320 

Regenerative Injection Therapies: Pricing is variable, all visit include a Consultation Fee of $175 (99214) + procedure fee ($140-$540 per joint). Detailed quote will always be provided at initial consultation.

HOW DOES YOUR FINANCIAL HARDSHIP POLICY WORK?

Our Financial Hardship Policy applies to any self-pay appointments or services, including our IV Therapy and Nutrient Injections.  ​

 

We value honesty and integrity in our interactions with our patients and do not require proof of income or financial resources. Instead, we operate on a financial hardship payment model to ensure our services are accessible to all individuals, regardless of financial circumstances. We will reverify this annually. We also ask that you inform us of any major income changes. Below are the three different income tiers that we consider:

Payment Tier One: Pay in Full
This tier is suitable for individuals who own their own home, vehicles, and other large assets. They have access to healthcare, can afford vacations annually, and may have inherited or will inherit wealth. They are employed and/or have a partner contributing to household income. Income is at least 3.5 times the federal poverty level or they have access to family wealth. Suggested income bracket is greater then $55k.

Payment Tier Two: 10% Discounted
This tier is for individuals with housing security through renting or owning. They may stress about monthly bills but can still pay them, they can take vacations every few years, and maintain a stocked kitchen with healthy foods. They have access to financial savings or family support when needed and own or lease a reliable vehicle. Income or equivalent is at least 2.5 times the federal poverty level. Suggested income bracket is between $30k and 55k.

Payment Tier Three: 20% Discounted
This tier is designed for individuals who may be employed but not sustainably compensated or are supporting a family with many needs. They are not able to go on vacation, or participate in activities that require memberships (gym, classes, clubs). They may lack secure access to healthy food and healthcare. They do not have family support or access to savings. Income is at or below the federal poverty level. Suggested income bracket is below $30k.
