HOW SHOULD I PREPARE FOR MY INITIAL CONSULTATION?

HOW DOES YOUR SLIDING-SCALE WORK?

HOW MUCH IS AN OFFICE VISIT IF IT IS NOT COVERED BY INSURANCE?

Our sliding scale applies to any self-pay appointments or services, including our IV Therapy and Nutrient Injections.  

We do not verify income but ask that you report honestly so that we can continue to offer sliding scale options to our patients who need them most. Likewise, we ask that you report any significant income changes to our office. 

If annual income is...

  • Greater than 50K, No discount

  • 30-50K, Tier 2 discount (10% off) 

  • Less than 30K, Tier 3 discount (20% off) 

**Please note, we do not offer double discounts. Sliding scale will not be applied to any specials we run. In addition, sliding scale discounts will not be compounded with discounts received from purchasing packages.

All fees are listed for payment at the time of service:

 

  • New Patient Appointment: $325 

  • New Patient Orthopedic Consult: $260 

  • IV Consult: $50

  • Long Follow-up: $250 

  • Standard Follow-up: $180

  • Brief Follow-up: $115 

  • Integrative Bodywork: $150

  • Pelvic Floor Therapy: $200

  • Regenerative Injection Therapies: Pricing is variable. See services for more details. Quote will always be provided at first consultation.

CAN A DOCTOR AT ARALIA HEALTH BE MY PRIMARY CARE PHYSICIAN?

Please note that neither Dr. Raven nor Dr. Cavalier function as a Primary Care Providers (PCPs). We recommend that you have a PCP outside of Aralia Health. This allows our doctors to focus on their specialty of Naturopathic Medicine during your office visit! 


We do not have the ability to accommodate patients for acute office visits. In addition, Naturopathic Doctors cannot prescribe certain medications in Montana. For these reasons, we highly recommend that you establish care with a PCP outside of Aralia Health.

WHAT IS YOUR CANCELLATION POLICY?

We charge a $50 cancellation fee if we do not receive notice of cancellation at least 48 hours prior to your appointment. This also applies to New Patient Visits.

 

If your New Patient Intake forms are not completed 48 hours before your appointment, your appointment will be cancelled and the $50 fee will be applied. Our forms are comprehensive and - for us to best serve you - it is important for the doctor to have adequate time to review them prior to your appointment. 

We are a busy practice and it helps us best serve all of our patients if we have adequate notice of any unexpected openings on our schedule. We appreciate your understanding! 

The cancellation fee may be waived in exceptional circumstances, however, this is at the sole discretion of Aralia Health. 

Register for a ChARM Portal, complete your New Patient Paperwork and complete Consent Forms. It is important to complete these forms as accurately and thoroughly as possible. Please include details of any medications or supplements that you regularly take, including dosage. If these forms are not completed 48 hours before your appointment, we will cancel your appointment. There is a $50 cancellation fee that is applied. Our forms are comprehensive and - for us to best serve you - it is important for the doctor to have adequate time to review them prior to your appointment. 

 

If you plan to utilize health insurance, we highly recommend that you also complete our Verifying Insurance Benefits worksheet. We are a small practice and do not verify insurance benefits for potential patients. It is up to you to determine this information! 

DO YOU BILL HEALTH INSURANCE?

Yes. We are currently in-network with Blue Cross Blue Shield, Aetna, Pacific Source, Allegiance, Cigna, MT Health Co-Op, and Missoula County. Although we are in-network, this does not guarantee that the plan you are on covers naturopathic physicians. If you plan to utilize health insurance, we highly recommend that you complete our Verifying Insurance Benefits worksheet. We are a small practice and do not verify insurance benefits for potential patients. It is up to you to determine this information! 

Please note, we do NOT bill insurance for any our IV Therapy and Nutrient Injections and some of our Additional Services. If you have insurance and elect to receive one of these self-pay services, we will ask that you sign an Advanced Beneficiary Notice agreeing to pay out-of-pocket. In these instances, we will not provide a Superbill. We will give you a price quote upfront and make sure there are no hidden costs.
